– Cough or shortness of breath
– A recent respiratory infection such as bronchitis, flu or pneumonia
– Left USA with 14 days (includes driving or cruise ship)
– Traveled to an infected area within the USA
– In contact with anyone in the last 14 days who has a confirmed case of COVID-19 OR is under investigation for the disease
